On your way into the park stop by Guest Relations and ask for some Great Service Fanatic cards. You just have to put the CM's name, your name, the date, and why. The CM takes care of the rest and gives it to their leader. A notation goes on the CM's record card and sometimes there are Great Service Fanatic drawings apparently. One of these cards is what City Hall has you fill out when you want to compliment a CM.

Are these cards really available for guests to walk out with?!? I've had trouble getting a card when I needed just one for a specific event.

I always thought that guests filled out applause-o-grams and then GR filled out a Great Service Fanatic card. Normally GSF are CM to CM.

Whenever I stopped by CH they always had me fill out a form with all the info on it, a short card would be much better, my handwriting is sloppy and my hand gets tired so the form might not be very legible. I spoke to some people at WDW a couple trips back that said they printed up some cards and would give them to the CM's that were really good. I know the compliments are recorded for them and I do want something that will give them credit where credit is due.

Applause-o-grams or Great Service Fanatic Cards are given to us and we put our Personnel Number on them and put them in a box. Then they show up on our record card as a Recognition Notation. Also we are entered into a drawing for a monthly prize.
